Location: Based in SE Portland; serves Portland Metro Area

Who You Are:

The goal of the Housing Transitions Program is to permanently house homeless self-identifying women living on Portland's streets, and to provide the housing retention and support services needed to help them remain housed.


What You Will Work On:

  • Conduct office hours at Housing Transitions Program to engage women in housing services.
  • Conduct site visits at various shelter locations as assigned by the In Reach team.
  • Work with shelter staff to receive referrals and work jointly toward client housing stability.
  • Assist homeless women in finding and securing housing.
  • Facilitate access to needed community services such as employment, health and mental health services, benefits assistance, and addiction treatment.

What You Bring:

Education, Certifications, and Licensure Requirements/Preferences

  • Bachelor's degree human services or equivalent lived and or worked experience required.

Experience Requirements/Preferences

  • Minimum 2 years experience in case management with individuals experiencing homelessness.
  • Experience providing case management, information and referrals and advocacy.
  • Committed to providing low barrier housing and support services to homeless individuals.
  • Understanding of the issues and challenges of homeless women.
  • Experienced in crisis intervention and de-escalation.
